Financial Policy:

We believe that good care for you and your family starts with good communication, and we have created this policy to help our patients understand the responsibilities that they and their families have for payment of our fees. Payment for our services is due in full at the time that those services are provided to you. This includes, among other things, co-pay amounts, program deductibles, earlier charges that remain unpaid and charges for services that we believe are not covered by or are left over as your responsibility to pay after coverage by insurance and government programs. We may also present charges to you by written statement via the mail. If we do this we expect each charge to be paid in full by return mail.

  • Insurance: In order to submit your insurance claims we need to have accurate up to date information. Please have your insurance card with you when you come to every appointment. We will need to make a copy of both front and back of your card to be able to send to the appropriate insurance address. If your insurance changes please let us know as soon as possible.
  • Co-Pay: If your insurance requires a co-pay, we will request that at the time of check-in for your appointment.
  • Deductible: If you have a high deductible and know you have not met the minimum you will be responsible to pay in full at the time you checkout. We will submit a claim to your insurance company which will be applied to your deductible.
  • Self-Pay: If you do not have insurance coverage you will be expected to pay in full at the time of service. You may pay with cash, check or credit card.
  • Sleep Consultations: If you are scheduling a sleep consultation, you will be responsible to contact your insurance company to see if your plan covers sleep medicine. If your insurance does not have a co-pay, you will be responsible to pay $200.00 at the time of check in which will be applied to the full spectrum of services for sleep medicine.
  • Medical Procedures: Minor surgeries and procedures are billed differently than routine office visits. You are responsible for knowing your health insurance benefits with regards to procedures so that you may be fully prepared for any charges you may be responsible for.
  • Lab Work: Charges for laboratory tests done in our office, immunizations, injections and other procedures performed by us are added to your bill. Services not performed here at Moscow Medical, such as special lab work and hospital services are billed to you separately by those providing the service.
